Supplied medical information may be passed to the School Nurse
for monitoring general health trends in school and for tracking
the onset of contagious illnesses within the school community. |

| Important Note: The form above should
only be completed and submitted by parents or guardians. Any
pupil found using this form with the intention to mislead
or gain advantage will have committed a significant breach
of school discipline. Such misuse will result in disciplinary
procedures being opened by Comberton Village College. If notified
details are judged to be inconsistent with known pupil information,
the college will contact the parents to confirm the pupil
absence. |

on the 'Notification of Pupil Absence' form to be confidential
information. Parents' e-mail and IP addresses may be used
for verification purposes but will not be published, distributed
or passed to third parties. As a processor of personal data,

things, that data is processed lawfully and that personal
